Established in 1977 The Gandara Center provides outpatient mental health and substance use services to the growing and largely unserved Hispanic community in Western Massachusetts. Gandara Center serves more than 10,000 people from all backgrounds per year at 45 locations across the state of Massachusetts.

As a Case Manager the individual will work within the Gandara Woman’s TSS team model, establishing open communication, support, and accountability among workers and consumers.  S/he is accountable to the Program Director, who is responsible for providing supervision and training.  The Direct Care 1 Staff is one who actively promotes the philosophy of the DMH Program, and maintains a high level of commitment to the program.


  • Assume enhanced responsibilities for planning and overseeing on and off unite activities, as well as provide advocacy services to clients.
  • Complete adult comprehensive assessment, individual action plans, group progress notes, weekly progress notes, discharge summary and aftercare planning.
  • Conduct intake interviews, administer and complete MIS forms and adhere to funding sources requirements.
  • Provide weekly individual counseling and also act as the group facilitator in regularly scheduled sessions which include peer discussion, didactic substance abuse seminars, and other therapy groups conducted on the premises.
  • Maintain direct supportive contact with residents to continually assess their individual needs, which can occur during stressful and crisis situations.
  • Responsible for notifying supervisory staff immediately in the event of a client crisis, suspected illegal activity or any behavior constituting and indent in the program.
  • Maintain clinical notes and reporting in accordance to established record keeping procedures.
  • Participate in all case consultation meetings, staff meetings and case conferences.
  • Administers drug/alcohol screens to residents when considered clinically appropriate.
  • Provides orientation of services to prospective clients, family members, and community interest groups.
  • Provide crisis intervention and serve as a client advocate.
  • Provide patient transportation to outside appointments when public transportation is not available or recommended.
  • Participate in in training programs (both external and internal); keep abreast of materials relative to chemical abuse, mental illness and treatment approaches.

Job Requirements:

  • Acquire and maintain current the necessary certifications mandated by the agency and funding sources.
  • Ability to work with individuals in recovery.
  • Organized and ability to work with minimal supervision.
  • Ability to master First Aid, CPR, Defensive Driving, and Fire Safety.
  • Demonstrates good verbal and written communication skills
  • Bilingual (Spanish/English) – Written, Spoken, Read
  • Must have availability of a reliable automobile to transport clients or perform other supportive services as necessary.


Minimum Qualifications:

  • High School/GED +2 yrs. experience.
  • Associates degree +2 years.
  • Bachelor’s degree =1-2 years’ experience preferred.
  • Experience providing substance abuse services.
  • Valid driver’s license and must meet the agencies requirements for driving history records.
