The Hampden County Continuum offers a large array of wraparound, 24/7 access services for families with youth ages 6 - 21 who are at risk of hospitalization, residential or foster care placement due to complex mental health and behavioral challenges. Access to out-of-home residential services for youth who can no longer be maintained safely in the home are also available.
